Form editor changes.

When app is protected by Username and Password: if User is registered in app system knows his email, so when he fills in form, Email field is already filled in for him, but only if Keep fields value after form submit checkbox is selected under Email field in admin of Form Editor gadget

Added "Send copy to me" checkbox in the end of the form and if User selects this checkbox, presses Submit btn, he gets forms copy on his email. In the same time app owner gets form result as before.
